We are located at the corner of 76th Street and Blumound Road - Suite #208.
Our office is in the same building as the BMO Harris Bank. Enter the building from the Northeast corner (directly across the street from Subway), and go to the second floor, Suite 208.

What should I bring with me to my first appointment?
So we can get to know you, there will be some medical history paperwork we need to complete prior to seeing you for the first time. Please arrive 15 minutes prior to your scheduled appointment to complete the paperwork.
We also ask that you:- Bring your Dental Insurance Card
- Bring a photo ID
- Know all current medical prescriptions you are taking

Have your previous dental x-rays sent to our office if the following applies:
- Full Set taken within the last 3-5 years, and Bitewings taken in the last year.
What if I want to cancel my appointment?
We have a 24 hour cancellation policy. All appointment cancellations must be confirmed over the phone at least 24 hours in advance to ensure you will not be charged.
What should I do in the case of an emergency?
If you are being bothered with tooth or mouth pain during our regular business hours, please contactus immediately.
